The Clinical Liaison RN for Home Infusion is responsible for fostering referral relationships and facilitating patient transitions to home infusion therapy by working with healthcare providers and facility staff. The role includes providing clinical consultations, education, and ensuring appropriate documentation for ordered infusion treatments. This position requires nursing licensure, clinical experience, and strong communication skills to support patient care within a home infusion pharmacy setting.

This position will be supporting Oregon Home Infusion Pharmacy.
The Home Infusion Clinical Liaison position is responsible for creating and maintaining positive referral relationships in order to increase Home Infusion Referrals within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and other individuals to assess appropriateness home infusion, facilitate patient care transitions onto service and ensure there is appropriate documentation for ordered infusion therapy. Consultative functions include, but are not limited to: providing consultation and informational presentations regarding home infusion therapy, and serving as a clinical resource and educator for ordering providers, facility staff, patients, families on Home Infusion Therapy.
