Essential Duties and Responsibilities:
Job Title: iHOPE Case Manager: This position requires a minimum of a Bachelor’s degree in Psychology, Sociology, Human Services or Nursing. One Year experience working with either children or adults may be required.
Qualifications: Bachelor’s degree from an accredited school. Submit to and pass successfully a criminal record check and drug screening test. Complete case management training and become certified in both adult and child case management within 6 months of employment.
Hours : Monday through Friday 8:00 am until 4:30 pm. Other times may be required with the job or adjusted by your supervisor.
This position reports directly to the iHOPE Team Leader.
Summary of Job Duties: The IHOPE Case Manager may only provide interventions to consumers with the Cumberland River Behavioral Health, Inc. The iHOPE Case Manager is a member of a multi-disciplinary team responsible for providing services to individuals between the ages of 15 and 30 who are experiencing symptoms of psychosis or who have been identified as being at risk for developing psychosis. The iHOPE Case Manager is to assist the individuals served to gain access to medical, social, educational and other support services.
